The Community Mental Health Team (CMHT) provide the link between primary health and secondary services. Each CPN is linked to 2-3 GP practices and undertake assessment and ongoing treatment of mental health problems for the adult population of their GP practices. Three teams, North, South and Central serve Gloucester City. Each team offers specialist medical, nursing, occupational therapy, social work or psychological therapy care and treatment to people with serious mental health illnesses.
