The aim is to prevent inappropriate or unnecessary admission to hospital or residential / nursing home, and to enable timely discharge from hospital. Care may be provided in the patient's home, or if remaining at home is temporarily not an option, they may offered a place in the The Intermediate Care Community Rehabilitation Team's specialist rehabilitation unit at Townsend House, Mitcheldean (see separate record).
