Comment:

The Intermediate Care Community Rehabilitation Team comprises care managers, physiotherapists, occupational therapists, rehabilitation assistants, etc. The purpose of the service is to maximise independence for people, mainly elderly, who need short term rehabilitation following a fall, stroke, infection, fracture, immobility - to enable them to regain their former independence. The aim is to prevent inappropriate or unnecessary admission to hospital or residential / nursing home, and to enable timely discharge from hospital. Care may be provided in the patient's home, or if remaining at home is temporarily not an option, they may offered a place in the The Intermediate Care Community Rehabilitation Team's specialist rehabilitation unit at Townsend House, Mitcheldean (see separate record).
