The Gloucestershire Adult Placement (GAPS) Scheme's function is to facilitate an individual Community Service to all adult service users. Care is provided in the approved carer's own home. This allows the service user to be fully integrated into the carer's family and local community, offering them an alternative to residential care - thus achieving the true meaning of Community Care and Valuing People with a range of needs.
GAPS is flexible and responds to the changing needs of the service user, and can facilitate individually tailored care plans. In March 2005, GAPS was registered by CSCI. The scheme works with Family Link and Fostering to approve all transition placements - these are for young adults moving on from Children's Services who still wish to continue living, or having short breaks, with carers.
The scheme now operates county-wide and is in the process of approving new carers to accommodate more complex placements as well as more straightforward ones. Care is available on a short break or longer term basis, and occasional emergency placements.
