The Horizons team offers a flexible home based childrens palliative care service for children and young people living in Gloucestershire, who have a life-limiting or life-threatening condition. They provide a specialist childrens palliative service, recognising each familys individual needs. The aim in Horizons is to promote the potential of each young individual, enhancing their quality of life as much as possible and ensuring the home based care they provide is of a high standard, whether it is nursing care, respite care or clinical psychology support.
The strains and stresses of caring for a child with complex care needs are immense and can continue for years. The Horizons team has developed a holistic model to support families and children with life-limiting and life-threatening conditions.
