Comment: | The aim of Intermediate Care Community Rehabilitation is to provide short term rehabilitation in a community setting or home environment for the Stroud and Berkeley Vale area. Therapy and care is provided as a method of preventing admission into hospital, residential or nursing home and promoting an early discharge from hospital. Therapy and care is provided on an individually planned programme from a few days to a maximum of six weeks. The team work as an integrated part of existing community services liaising with and referring to voluntary and statutory agencies. The therapeutic team includes a physiotherapist and occupational therapists amongst others. The service is useful in the case of falls, fractures, minor strokes, immobility or severe pain. The team work with people to enable them to repair confidence and independence by providing eqipment and therapy.
