Comment: | Rayners Hedge is run by the Buckinghamshire Primary Care Trust (PCT), for people aged over sixteen years old who have a physical neurological disability. The rehabilitation staff work with individuals and their families to clarify their needs and preferences. They will then advise, facilitate and help them to optimize their independence and quality of life. This is a multi-disciplinary team. All referrals must be approved by the patient's GP. |
