The Gloucestershire Heart Failure Service sees patients referred by GP's for an echocardiogram to confirm or refute the presence of heart failure. If confirmed, an appointment is made with a GPwSI and a management plan is formulated which the Heart Failure Nurses carry out - liaising with the patient's GP, Practice Nurse, District Nursing Team as appropriate.
The Heart Failure Specialist Nurse Team also sees patients following Hospital Admission for heart failure - seeing them initially at home and then later at the clinic as their condition improves. The Cardiologists also refer directly to the service; nurses from hospitals and community nurses can also refer, provided the patient's clinician is informed and agrees.
Patients and carers can also call the patient helpline, which is staffed from 9:00am to 5:00pm, Monday to Friday.
