Gloucestershire Social Services provides a range of services for people who need care to remain in their own home. Some of these services are provided directly; others are bought in from voluntary and private contracted agencies.
Who is eligible for assessment?
+ Older people;
+ People with mental health problems, learning disabilities, physical disabilities who are at risk of not being able to continue living independently in the community or not being able to return home from hospital.
Care may also be provided to families and children with disabilities who require support. Initial contact through a customer services officer who will refer the client to an assessor social worker OT etc, a care plan will then be drawn up and passed to home care or an independendent provider, for the home care to start.
