Comment:

The aim of Cheltenham and Tewkesbury Intermediate Care is to provide short term rehabilitation in a community setting or home environment for the Cheltenham and Tewkesbury areas. Therapy and care is provided as a method of preventing admission into hospital, residential or nursing home, and is provided on an individually planned programme from a few days to a maximum of six weeks. The team works as an integrated part of existing community services liaising with and referring to voluntary and statutory agencies. The team includes physiotherapists, occupational therapists, CPN's, Social Worker and nurses amongst others. The service is useful in the case of falls, fractures, strokes,immobility or severe pain. The team work with people to enable them to rebuild confidence and independence by providing equipment and therapy.
NB. Referrals are mainly from GP's District Nurses, A departments and Social Service professionals and are not usually taken direct from the public.
