Comment: | The Community Support service, though provided by Stonham, is not exclusive to tenants living in Stonham properties. The service visits people (aged over eighteen) with mental health issues or a learning disability outside of standard working hours to enable them to live as independently as possible. There is also an on-call facility between the hours of 10:00pm and 6:00am for crisis support.
The service is user led, and the level of support - practical or emotional - depends on individual needs; it can help people with socialising - to ease someone's isolation, managing money, access to work and training, tenancy support, using public transport and meal planning. Short term support can also be used to help someone gain confidence with shopping.
Visits can also be used as a respite facility for people who satisfy the service criteria. This lets the carers have a break while the person cared for gets individual support to develop their own interests and meet their own needs.
Referrals are usually through a social worker or care co-ordinator. An application form has to be completed to receive the service. |
