Comment: | Gloucester Intermediate Care Team is a joint NHS and Adult Social Care funded community service. It works closely with other services and comprises care managers, physiotherapists, occupational therapists, rehab care co-ordinator, rehab assistants and a nurse. The purpose of the service is to maximise independence for people, mainly elderly, who need short term rehabilitation following a fall, stroke, infection, fracture, immobility - to enable them to regain their former independence. The aim is to prevent inappropriate or unnecessary admission to hospital or residential / nursing home, and to enable timely discharge from hospital. Care may be provided in the patient's home, in intermediate care beds or daycentres, and is available up to six weeks free of charge. |
