Age Concern Gloucestershire offers a hospital after care service. The service is for older people who may have lost confidence following a stay in hospital and would benefit from help and support settling into their own home. Trained volunteers will help with simple household tasks, shopping etc.
The service is for four weeks; however some clients may benefit from a further four weeks with the social rehabilitation officer.
The area covered by this service is Gloucester, Churchdown, Brockworth, Stroud and Cheltenham.
